CONTRACT TRACING - SUMMER HOLIDAY
Please complete and submit a copy of our Coronavirus (COVID-19) Contact Tracing Form for any pupil or employee of White woods Primary Academy Trust who has tested positive for Coronavirus (COVID-19) and developed Symptoms on :
Friday 16th July
Saturday 17th July
Sunday 18th July
The main symptoms of COVID-19 are:
- A high temperature - this means you feel hot to touch on your chest or back (you do not need to measure your temperature)
- A new continuous cough - this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than ususal)
- A loss or change to your sense of smell or taste - this means you've noticed you cannot smell or taste anything, or things smell or taste different to normal
If the person who has tested positive for Coronavirus (COVID-19) did not develop symptoms please complete and submit a copy of our Coronavirus (COVID-19) Contact Tracing Form if their test was taken on:
Friday 16th July
Saturday 17th July
Sunday 18th July
Please complete and submit a copy of our Coronavirus (COVID-19) Contact Tracing Form by Thursday 3rd June at the latest.
